New York State Dental Foundation's Deans Award

Ensuring a growing and sustainable oral health work force is crucial to expanding the availability of oral health services for all New
Yorkers, particularly as the needs of underserved and culturally diverse communities expand. To help address this critical issue,
the New York State Dental Foundation has established a Deans Award.

The New York State Dental Foundation Deans Award is awarded annually to junior dental students or postdoctoral trainees who
have demonstrated an exceptional level of achievement as measured by:

- outstanding academic performance;
- demonstrated commitment to enhancing and improving the oral health of underserved populations, and
- membership in the American Dental Student Association.

The candidate for the NYSDF Deans Award, valued at $5,000, must be nominated by the dean of each of the following academic
dental institutions: Columbia University College of Dental Medicine; New York University College of Dentistry; State University of
New York at Buffalo; Stony Brook University, and the University of Rochester Eastman Dental Center.

The dean of each academic institution can nominate one candidate per year, unless special circumstances lend themselves to the
nomination of two candidates. In any event, each academic institution will receive a maximum of $5,000 per year for this award.

The NYSDF Deans Award will be presented to each recipient at his/her respective graduation ceremony.

Nominations should be sent to: Laura Leon, director, NYSDF, 20 Corporate Woods, Suite 602, Albany, NY 12211, no later than 1
February 2008.

Instructions:

Deans should complete the application and mail it so that it arrives at NYSDF offices no later than 1 February 2008. By 1 March,
deans will be notified of acceptance of their choices.

Each application should include a cover or general information page and a letter of recommendation from the nominator. Such
letter must attest to why the nominee is the ideal recipient of the NYSDF Deans Award.
