





| 2008 NYS Dental Foundation Give Kids a Smile Award The New York State Dental Foundation has established a $5,000 award to be given annually to a community health center or service program which seeks to build, expand or enhance education, access and/or capacity with respect to oral health care for children. The New York State Dental Foundation Give Kids a Smile Award was inspired in part by the American Dental Association’s efforts to help children get the dental care that they so desperately need and to raise awareness that our children deserve a better health care system that addresses their dental health needs. The Foundation will be accepting nominations through 1 April 2008. NOTE: You must either be a member of the New York State Dental Association or have a letter of support from a NYSDA member in order to submit nominations. Nominees must be not-for- profit programs. In nominating a community health center or service program, consider the following questions:
Nominators must complete the attached nomination form, which includes a written nomination statement and letters of support from at least one other organization or individual. Form will include name of community health center or service program, title, mailing address, phone and email, contact person. Also, please include basic contact information of the nominator. Must include a written nomination statement detailing why the nominee should receive the award, no longer than 2 pages, and MUST explain HOW the nominee meets the criteria to merit recognition. If appropriate, a letter of support from a local dental organization/component society should be included with the application. Click here to download the nomination form. |
