Since 2007, the New York State Dental Foundation
(NYSDF) has given more than $100,000 to community
health centers or service programs as part of its Give Kids
a Smile Award (GKAS) program. NYSDF recognizes
programs which build, expand or enhance education,
access and/or capacity with respect to oral health care for
children. The NYSDF GKAS Award helps to raise
awareness among the community at large our children
deserve good oral health.

We are now accepting applications for the 2012 GKAS
Awards. Winning applicants will be awarded an amount up
to $5,000, as determined by the NYSDF Board of Trustees.
The deadline for nominations is Friday, 13 January 2012.
Winners will be announced in the spring.

In nominating a community health center or service
program, consider the following questions: Who is setting
higher standards of oral health care and quality to better
meet the challenges of the future? Who is driving
innovation in oral health care delivery and improving oral
health outcomes within our communities? Who is
consistently demonstrating the type of leadership and
advocacy that is moving New York State in new directions to
meet the oral health care needs of children? Does the
potential nominee target education, screening and
prevention, and access objectives in its parameters?

To apply, complete the attached nomination form, which
must include:

-        a one page written statement detailing HOW the
nominated program meets the criteria to merit                     
recognition;  
-        a detailed breakdown of what nominee intends to do
with the Award money;
-        a detailed budget outlining how GKAS Award money
would be used;
-        a list of other funding sources available to support the
nominated program.
-        letter[s] of support from at least one other
organization of individual

Incomplete nominations will not be considered.

In addition, all GKAS Award winners must provide a
follow-up report, six months following the disbursement of
prize money, detailing the specifics of how the money was
used
